Process for submitting digitally :

1.) Call or email Dr. Ruof for email submission instructions or digital account set-up with Amplify PACS. 

2.) For Amplify PACS account set-up: A support technician will call your office at your convenience to set-up your account. The technician will  instruct you and your staff how to digitally send images and reports. 

 

Process for submitting films or CDs for consultation:

1.) Complete the Diagnostic Imaging Consultation Form

2.) Mail the images (films or CD) and completed Diagnostic Imaging Consultation form to:

3.) A PDF of the report will be faxed within 24 hours of receipt of the images. Additionally, the original report with the films/CD will be returned promptly via mail.

 

Other information:

1.) An invoice detailing monthly charges will be mailed on the 1st of each month. Please submit check to:

2.) STAT reports or telephone consultations are available at no charge, if requested.
